This is an adapter that I got together with my phone. I don't know if I ever used it with my phone, but I use it often with my laptop. Maybe I used it once or twice when charging my phone off my tablet, but with the adapter plugged in to the tablet (yes, this is a thing you could do if your devices support it, you don't need to thank me for the tip).
Recently I've been using it a lot, because my work MacBook doesn't have any USB-A ports. On one hand I like it because it's useful, on the other hand I would prefer if everything was using USB-C already and I didn't need it anymore. It's short and blocky, and the A port is a bit off-center:
